ALL FEES and COSTUME PAYMENTS ARE NON-REFUNDABLE
Payment by: CASH, DEBIT, CHEQUES (3 postdated at registration)
or CREDIT CARD: VISA or MASTERCARD (2.5% fee added)
LATE CHARGES: $20.00 if payment not received by dates due. $20.00 charge for any returned cheque.

1 class per week......... 3 equal payments of $160.00 (Sep 1/10, Dec 1/10, Mar 1/11)
2 classes per week...... 3 equal payments of $296.00 (Sep 1/10, Dec 1/10, Mar 1/11)
3 classes per week...... 3 equal payments of $418.00 (Sep 1/10, Dec 1/10, Mar 1/11)
4 classes per week...... 3 equal payments of $530.00 (Sep 1/10, Dec 1/10, Mar 1/11)
COSTUME FEES (HST INC.)
COSTUME payment due: NOVEMBER 20TH, 2010 - $80.00 (total price) per costume.
CONDITIONS OF ENROLLMENT
1. No refund will be given for missed classes. Classes unavoidable missed may be made up by attending any
other suitable class in the same term. CANCELLED CLASSES: In case of bad weather, please check our website
www.barbaraashleydance.com or listen to the local radio stations for a message.
2. Classes are subject to change depending on the number of applicants. Any class with insufficient
registration may be cancelled and an alternative given. Class times may alter during the first 4 weeks of the
school year.
3. Students with unpaid term fees or costume fees will not be permitted in the Recital. Students must attend
all rehearsals and shows held at the Arts & Culture Centre. The selection of music and costumes will be the
sole decision of the director.
4. No street shoes will be permitted in the studio. Parents and students are asked to remove all shoes
and boots and leave them in the porch.
5. ALLERGY ALERT: The following items are not permitted in the studio: nuts, peanut butter or oil, juice boxes,
chocolate, fish.
6. NO CHEWING GUM ALLOWED ANY WHERE IN THE STUDIO!
7. NO CELL PHONES ARE PERMITTED IN CLASS!
"SAFETY FIRST" DRESS CODE
PROPER DANCE FOOTWEAR AND DANCE CLOTHING IS REQUIRED AT ALL TIMES. To avoid injury, students
will NOT be permitted to participate in class wearing socks, bare feet, baggy pants or loose clothing. NO
JEWELLERY. Hair must be worn neatly off the face in a bun, braid or pinned back.
Students must wear proper dance clothing to all classes:
PRE-BALLET & BALLET: Pink or black bodysuit, black tights, pink ballet slippers, ballet skirt.
JAZZ & JAZNASTICS: Unitard or bodysuit and stretch pants. No baggy clothes. Appropriate footwear.
TAP & HIP HOP: T-shirts, Capri or bicycle length pants. NO LONG PANTS TOUCHING THE FLOOR.
Appropriate footwear.
Please mark all items of clothing and footwear, etc. with student's name. The studio is not responsible for lost
or stolen items.
